DISTRICT # 3 BOARD MEETING MINUTES for Thursday, July 12, 2001

Meeting called to order at 1938 hours by the Chair.

Motion to accept the minutes from June 14, 2001 made by Lee Jones and seconded by Maureen Leahy. Motion accepted with no corrections/amendments.

Introductions:

Correspondence:

-Notice from Sean Collins (Essex Rescue) regarding Champlain Valley Fair Coverage. If squads are interested in providing fairground and first aid station coverage, please contact Rob Lunn at (802) 527-1775 or via email at  edelweiss@together.net

Treasurer’s Report:

-Arliene advised that district dues were sent out. Debbie inquired if dues are adequate to cover expenses.

Medical Direction:

-The intubation recertification scheduled for July 17 has been postponed until August or September. Drs. Elliott, Eisenger and Weimersheimer are waiting for Dr. Misselbeck to give a date.

-Kathy Karg will be doing case review on July 17 as well as reviewing Epinephrine delivery other than EPI pens (i.e. Hollister-Steer syringes and pending approval from Medical Direction, EMT-I’s may be able to draw out 1:1000 EPI from 1cc ampules)

<Dr. Weimersheimer arrives> He doesn’t have a problem with using ampules; since max dose is 1 cc, low error rate. Dr. Weimersheimer to check with Dr. Elliott to make sure ok.

-Gil advised that hospital doesn’t carry Hollister-Steer syringes but they do carry the ampules of EPI. He will look into obtaining "practice " ampules for squads to practice.

Fletcher Allen/MHCIC:

-Case review to be held July 17, 2001 at Burgess 1900 hours

-Gil encouraged squads to pick up copy of the "Fletcher Allen SUN" (hospital newsletter) to keep up with the construction. As of Monday, July 16, the existing entrance to the ER will be no longer. Patients will now enter via the walkway in the ambulance bay to the triage/admitting area. Gil will ask the Radio Room to post construction changes/updates and to make sure incoming ambulances are aware of the changes.

-Lee Jones brought up a recent incident where he was on the ambulance and realized that his rig couldn’t back into the bay and unload the patient because of the lockers. If squads aren’t at the hospital frequently, they may not be aware of the changes.

-Greg will scan copy of the "SUN" for squads to see the phases of construction, keep updated on the changes.

-Gil reminded squads to communicate to the ED any needs during this time.

-Debbie encouraged squads to keep their drivers with the rig in case it needs to be moved.

-Dan Greaves inquired about where to park when move out of bay since the 3 designated spaces are often filled with FACT/Lamoille rigs.

MCI:

Not present

Policies and Procedures:

-There was a discussion on the DRAFT backup policy for motor vehicle accidents, MCI’s brought up by Maureen. Previously the crew chief on the primary ambulance was the only one who could cancel backup, Fire etc. The new policy changes include " … Officer in Charge of the scene" and "…in a time frame of less than four minutes…." The phrase "Officer in Charge of the scene" was written because the person in charge differs by squad and this way it could be adapted to fit the different district services. The four-minute time frame was added to give squads some leeway with the extent of the response and also reduce the number of units going code 3.

-An example was brought up concerning an MVA that happened on East Ave (Burlington) earlier in the day. Engine 3 dispatched UVM Rescue because it was right by their quarters. According to the new policy, UVM can cancel Engine 3 since UVM would be the Officer in Charge of the scene. The question was then asked what brought about this change in policy. Maureen advised it was a question of liability. Tanner and Lee both reminded all present that this is an EMS policy and not a Fire or Police policy. Further discussion ensued about the term "Officer in Charge of the scene" and its ambiguity. Lee advised that he would change the old policy to read "The cancellation of any ambulance…is at the discretion of the primary response agency" and "The primary response agency responding to a multi-casualty incident will advise the Fire Department…" and send it out via the listserv for discussion, comments.

-Lee also presented changes to other policies/procedures to reflect what we actually do. These changes include:

Aircraft disaster at Airport (Appendix S) - replace IFR to Williston VSP with Williston Rescue to VSP.

Defibrillation run report policy - remove "Squads that have Zoll Semi-Auto Defibrillators must download the memory cards…" and "squads with other types of Semi-Auto Defibrillators will need to leave the appropriate data storage device…" Since this does not happen anymore, squads will leave the state defibrillation run report and the Vermont EMS Incident report at the hospital.

Radio Communications - In Cardiac Arrest Update, remove #5 "If EMT-D level: initial rhythm…" and renumber accordingly. Maureen suggested keeping the # of shocks and changes in condition. Also changed MCHV to FAHC where appropriate.

Motion to accept changes made by Maureen Leahy, second by Kevin Wood. Motion passes.

Logistics:

Debbie advised that South Hero Fire Department would be responding with South Hero Rescue to US Route 2 for distance where South Hero Rescue is primary ambulance.

Heavy Rescue/Fire:

Kevin Williams brought up that BFD recently purchased airbags for extrication. Engine 1 will be the responding engine for calls outside of the city.

E-911:

Steve Fleming advised there is a meeting next week.  Maureen asked to be taken off list, as she can no longer attend meetings because they are on Tuesdays.

Training:

Keith Robinson is looking to coordinate an EMT class with an alternate schedule (such as 1 night a week for ? months, every other weekend). Any feedback would be appreciated.

New Business:

Malletts Bay Fire Department member Peter Igneri is looking to challenge EMT-B exam and seeks support from District #3, Medical Direction. Pete is currently a Physician’s Assistant at Colchester Family Practice and has been with MBFD for a number of years. Concerns were raised about his familiarity with equipment and protocols. Dr. Misselbeck is aware of the petition. Greg Thweatt suggested that Dr. Weimersheimer speak with Dr. Misselbeck to confirm he is ok with this.

Tanner also noted that MBFD is seeking to buy Defibrillator and that there is a new Zoll representative for this area.
